CRI's Transitions Project was developed out of an increasing need for individuals of all ages with significicant disabilities to live independently with the support of services in their communities. For the last three years, the Transitions Project has been developing processes for relocating individuals with disabilities from institutional environments into their communities, affording them opportunities for independence and community participation. Working in partnership with physicians, social workers, discharge planners, nursing homes and family members, Transitions Project staff and the person with a disability will develop a plan, set goals and explore resources in order to bring about a smooth transition into the community. Greater independence brings greater responsibility. Individuals will be involved in daily decision making, reviewing choices in housing, interviewing and choosing a Personal Assistant, forging new friendships, attending support groups, and exploring employment opportunites and assistive technology possibilities.
